Our 10 year Integrated Care Strategy sets out the priorities for integrated working across health, care, local authority and the wider voluntary and community sector. This strategy is broad and covers the life course, from maternity to end of life care, and everything in between.

These priorities are based on the needs of local people, using data, insights and engagement – What we have heard from our local communities.  

To ensure that we are able to deliver meaningful improvements in these areas we have worked together to gather an evidence base and community insights to develop and a range of ‘theme’ strategies, for clinical or disease specific areas, and enabling functions, such as our Local Maternity and Neonatal System Strategy or Digital, data and Technology Strategy. 

These strategies provide the detail and focus for each ‘theme’ over the next 5 years, covering outcomes to be delivered through local plans across Herefordshire & Worcestershire and within each county. We are pleased to make these available here, so you can see the ambitions we have to improve access and outcomes for local people whilst also reducing health inequalities.
