The Local Transformation Plan for children and young people's mental health and emotional wellbeing in Herefordshire and Worcestershire.
This document describes how, as a local system, we are improving the mental health and emotional wellbeing of all children and young people across Herefordshire and Worcestershire. Our Local Transformation Plan is reviewed, refreshed and published annually, in line with the requirements of the NHS Long Term Plan. This latest version of the Herefordshire and Worcestershire Transformation Plan for Children and Young People's Mental Health and Emotional Wellbeing was published in October 2023.
The delivery plan will be owned by the Worcestershire Children and Young People’s Emotional wellbeing and Mental Health Partnership Board and the Herefordshire Children and Young People’s Emotional wellbeing and Mental Health Partnership Board. These partnership boards will hold partners to account for the actions, the boards report into the Children and Young People’s Strategic Partnership Boards in each local area. This delivery plan will be refreshed for 2023/24.
