Driving the shift upstream to more prevention and best value care in the right setting- 2025/26 to 2029/30
This NHS Five Year Joint Forward Plan (JFP) has been produced by NHS Partners across Herefordshire and Worcestershire. This is a 5 year rolling plan, with the first JFP published summer 2023. The plan is then formally refreshed on an annual basis, moving it forward a year and reflecting on what we have delivered together over the previous 12 months.
The plan describes the shared priorities that partners will collectively work on over the next five years in response to the Integrated Care Strategy and Joint Local Health and Wellbeing Strategies.
The strategic intent is to collectively drive the shift upstream to more prevention and achieve best value care in the right setting. Delivery of the plan, builds on existing programmes of work. The main document provides the strategic overview for the JFP and you can find more detail in Appendix 2: Cross cutting themes, about our commitment to early engagement and ongoing dialogue with people and communities.

*During March 2025 the JFP has been reviewed and a refresh completed to update on delivery during 2024/25 and focus for 2025/26, moving the plan forward a year. The overall strategic intent remains the same. In line with national planning guidance, a full refresh will be completed later in 2025/26 following the publication of the NHS 10 year plan and the requirements within it. The 2025/26 refresh will be published above following formal approval by NHS partners Boards and equivalent governance meetings for General Practice during Q1 2025/26.
